Supporting Community Integration for Persons with Disabilities

ILRC assists with the transition of people with disabilities from nursing homes and other institutions to home and community based living. In addition, we provide assistance to those at risk of entering an institution and help youth with disabilities integrate into their community.

Your Transition to Independence

We will work with your Managed Care Organization (MCO) to assist with transitioning from a nursing home or other facility. We can also assist through our Medical Equipment Recycling Network (MERN) to provide equipment and supplies for a more successful transition.

When a young adult is transitioning from school, we can assist them with applications for Social Security SSI, Medicaid, Housing, and Housing Choice Voucher program, as well as other services through our IL Department that will assist you in becoming as independent as you need.
